Printable forms and applications are available to complete and return to the credit union at your convenience. Completed forms and applications may be mailed or delivered to the Amherst FCU office, faxed to 716-634-1272, or emailed to memberservice@amherstcu.org All printable forms and applications must be completed with your signature. Please accompany Visa® Credit Card and Home Equity Line of Credit applications with income verification.
